USA/MTA Sick Leave Bank Application

Please note that new members are automatically enrolled, but cannot access the SLB until after their probationary period. USA bargaining unit members must participate in the Sick Leave Bank (SLB), in order to apply. Members who currently are not active participants in USA’s SLB program and wish to enroll, are required to donate a full day, and have a minimum of (3) sick days accrued in order to join.  

An open enrollment period runs each October. If you have immediate questions about accessing the SLB, please contact via AskHR.

*It is very important to make sure to submit FMLA paperwork. FMLA is there to protect your job and is required.*
