You need to be a member of USA and a member of the sick bank in order to apply. USA members who currently are not members of the SLB and want to enroll, need to donate a full day, and have 3 sick days accrued in order to join. New members are automatically enrolled but cannot access it until after their probationary period.
There are 3 open enrollment periods, February, May and October. This will allow members to join the SLB, current SLB members to voluntarily donate additional days. If you have questions about the SLB you may contact Randy Gryzch in Human Resources at 413-545-1473.
One thing to remember – make sure you get your FMLA paperwork in order. FMLA is there to protect your job and is required.
