USA members must participate in the ‘USA Sick Bank” (SLB) in order to apply. Members who currently are not members of the SLB and want to enroll, are required to donate a full day, and have a minimum of (3) sick days accrued, in order to join. Please note that new members are automatically enrolled but cannot access the SLB until after their probationary period.

It is very important to make sure to submit FMLA paperwork. FMLA is there to protect your job and is required.
